If your Insurance plan requires you to get a referral to visit an ophthalmologist (a specialist), please ensure that you get the referral from your Primary Care Physician (PCP) before you visit us.
Routine Eye Exams
If you qualify for a periodic routine eye exam, please ensure that you are eligible for the plan (by calling your insurance company) before making an appointment.

The final responsibility for ensuring that the insurance plan that you buy or are enrolled in considers our doctors in-network for our locations rests with you. We will not be responsible for validating the accuracy of your plan participation and details because we are not privy to all the details.
Our insurance validation checks are basic and do not cover all the nuances of today's increasingly complex insurance plans.
Insurance companies can also retroactively determine that you were not covered in the past and consider you liable for payments to us. While this scenario in not that common, it does happen, so please confirm very clearly what your insurance will or will not cover when you visit us.
Co-Pay, Deductibles, Co-Insurance
Co-payments for your insurance are to be paid in full at the time of your visit and we accept all major credit cards, cash and checks.
Insurances are increasingly passing the costs onto patients through various names such as coinsurance, deductibles etc. In exchange for lower premiums, patients are increasingly required to pay the doctors more money directly.
Please be aware that you may be responsible for out of pocket costs despite having insurance. The statements for these costs will be mailed to you and these payments are to be made to the Eye Center directly.
