Insurance Information

If your Insurance plan requires you to get a referral to visit an ophthalmologist (a specialist), please ensure that you get the referral from your Primary Care Physician (PCP) before you visit us.
Routine Eye Exams
If you qualify for a periodic routine eye exam, please ensure that you are eligible for the plan (by calling your insurance company) before making an appointment.
Co-Pay, Deductibles, Co-Insurance
Co-payments for your insurance are to be paid in full at the time of your visit and we accept all major credit cards, cash and checks.
Insurances are increasingly passing the costs onto patients through various names such as coinsurance, deductibles etc. In exchange for lower premiums, patients are increasingly required to pay the doctors more money directly.
Please be aware that you may be responsible for out of pocket costs despite having insurance. The statements for these costs will be mailed to you and these payments are to be made to the Eye Center directly.

We frequently change our participation with insurance plans and insurance plans also frequently and without notice change our participation in their plans. In case your plan isn't listed here, please give us a call and then check with your insurance plan to verify whether the doctor you wanted to see is "in-network" for the Framingham office.
